Overview

In constrictive shoes, the big toe is forced to bend toward the second toe and the first joint of the big toe is moved out of place. To compensate for the realignment, the outside of the joint is increased in size. Tendons then begin to pull the toe into an abnormal position. Over time the change in position becomes painful and permanent. The change in position also causes the mechanics of the toes and foot to be affected. The joint at the base of the big toe carries a lot of weight when walking or running. In a normally shaped foot the position of the big toe helps create a wide base of support and stability. A foot that has had the big toe bent toward the second toe will tend to roll inward. This abnormal pronation, along with the ill-fitting shoes will make the Bunion even worse. If a person has a foot anatomy that is prone to Bunions, wearing footwear with a too-narrow toe box will accelerate the development of a Bunion. Wearing footwear with a wide toe box may help prevent or at least delay the development of Bunions.

Causes
Bunions are most often caused by faulty mechanics of the foot. The deformity runs in families, but it is the foot type that is hereditary, not the bunion.Certain foot types make a person prone to developing a bunion. Injury can be a cause, especially if just one foot is involved. Although wearing shoes that crowd the toes won?t actually cause bunions in the first place, it can make the deformity progressively worse. That means you may expererince symptoms sooner.
SymptomsAlteration in alignment of the first toe. Pain in the 1st toe joint with movement. Restriction in range of demi pointe. Inflammation of the 1st toe joint. Rotation of the big toe so that the nail no longer faces upwards. Occasionally bruising of the toe nail occurs.

Diagnosis
A doctor can very often diagnose a bunion by looking at it. A foot x-ray can show an abnormal angle between the big toe and the foot. In some cases, arthritis may also be seen.

Non Surgical Treatment
Apply a commercial bunion pad around the bony prominence, use only non-medicated pads. Wear shoes with a wide and deep toe box. You should be able to "dimple" the leather over your bunion. Avoid all high heeled shoes. If your bunion becomes painful red, and swollen try elevating your foot and applying ice for about 20 minuets every hour. If symptoms persist, consult your podiatrist or physician.


Surgical Treatment
If your bunion is symptomatic and causing you persisting and troublesome symptoms then surgery should be considered. There is no correct amount of pain or inconvenience which a bunion may cause which warrants surgery. Symptoms which a bunion causes are generally subjective, and what is a problem in one person?s view will not be a problem in another's. For bunion surgery to be successful (correcting the deformity and losing the symptoms) the mechanical factors driving the deformity should be overcome. Bunion surgery should replace the 1st metatarsal closer to the 2nd thus reducing the width of the foot, and also realigning the tendons and reducing their deforming forces. These principals of bunion surgery are well demonstrated by the following x-rays which shows how a Scarf osteotomy has achieved this aim.

Prevention
Here are some tips to help you prevent bunions. Wear shoes that fit well. Use custom orthotic devices. Avoid shoes with small toe boxes and high heels. Exercise daily to keep the muscles of your feet and legs strong and healthy. Follow your doctor?s treatment and recovery instructions thoroughly. Unfortunately, if you suffer from bunions due to genetics, there may be nothing you can do to prevent them from occurring. Talk with your doctor about additional prevention steps you can take, especially if you are prone to them.
